The dentist scene in Marathon Man is something I have never forgotten and have sympathy pains over just thinking about it!
Also...Tom Hank's absessed tooth in "Castaway". How bad does it have to be to knock it out with an ice skate and a rock?

There's Nick Nolte's little bit of oral surgery in Affliction, and while it ain't normal dentistry, I'd also suggest the scene in The Thing (Carpenter version) where the fellow has teeth in his stomach -- those were some uncomfortable teethers.

BTW. I just got one a me molars yanked this very last Monday. And thanks to my insurance I got a prescription of a dozen vicodin filled for 3.50!WooHoo!
